UMLS API #223

This PR makes the following changes:

Adds an API module for talking to UMLS endpoints
Cuts over to the responses library for mocking (due to requests_mock not well supporting streams)

Out of scope of this PR is integration with the CLI, which will come in a future PR once there is a builder that uses it.
