ModRewrite conditions are case senstive #160

What steps will reproduce the problem?
1. add a condition with the [NC] flag
2. the condition matches all cases, regardless of the flag being missed.

What is the expected output? What do you see instead?
by default, it should be case sensitive... if the flag is present, it should be 
case insensitive.

What version of the product are you using? On what operating system?
4.0.4
