Document all functions #7

Properly document each function (think the kind of stuff that's in
dbi/MySQL.php) as well as give each .php file the proper heading

Javadoc format is now being used for all of the DBI abstract interfaces.  It's 
a good
start, I suppose.

I think at this point all we might have left to document is post-common, which

pretty crazy in retrospect.
