These are some comments about the document formatting:
There are some inconsistencies in the titles of the sub-sections. For example, in some parts it's “syntax” and in others it's “interface syntax”. This is similar for “semantics" and "interface semantics". I'm not sure if that depends on the type of module or not.
In the first few modules, the definition of the access programs is under "Access routine semantics", while in the last few modules the definitions are under "Semantics". I would make the titles consistent by using the former title.
In section 6, per the template, "Environment variables" should be a sub-section of "Semantics". So the number should be "6.4.3" instead of "6.5".
