mediate crash reporting #161

Enhance crash handling dialog to allow diagnostic info to be sent to @hm
site. We will need a corresponding PHP handler on the site to allow info to
be sent via HTTP post. Ideally this should include mediate log file, and
allow user to enter a description of what they were doing prior to the crash.
