Need to add support for custom refresh rate #162

DirectX default refresh rate tends to look good on monitors but not TV.
Need to add support for specifying a custom refresh rate in the display
setup screens of our theme (already added support to store & parse this
value in Preferences file, but is not yet utilised).

May also add support for specifying presentation interval - using interval
of IMMEDIATE under Vista results in much faster rendering.
