Format code with Elixir 1.6-rc.0 #428

add .formatter.exs
run mix format to resolve minor warnings like the below

Note: although I used Elixir 1.6-rc.0 to perform this, an upgrade in the Elixir version is not a part of this PR.

warning: variable "repo" does not exist and is being expanded to "repo()", please use parentheses to remove the ambiguity or change the variable name
