I am getting almost the same error with ss_get_by_ssh.php
in my cacti log i have:

Please see if you can figure it out by going through the Cacti
debugging steps, which are posted on their forums and their user
manual. If the answer is that the scripts or templates need to be
updated, please file an issue report with the necessary changes. I
would be surprised if that is the case. It is usually something to be
done inside of Cacti, such as clearing the poller cache.
The scripts themselves are obviously working, as shown in the original poster's
output.

Please follow the instructions at
http://www.cacti.net/downloads/docs/html/debugging.html to resolve your
problem. The
script works fine from the command-line, thus, it is not a problem with the
script.

I think I found the problem.
I guess that is because the MYSQL user name password variables are not given
