Reported Content Infrastructure

We need the infrastructure to actually manage reported content.

• there should be 1 standard page that allows a user to fill out a "report" form (or maybe a UI pop-up, whatever it is.)
• everything should be reportable.
  • projects, comments, users, courses, assignments
• Here's what the report page should track:
  • resource (obviously)
  • reporting user
  • time reported
  • It should have a dropdown for severity
  • dropdown for the reason
  • a box for additional comments
  • internal method of tracking status
  • Commendable? (internally...so that mods could discuss things?)
