Better note recording for badges and applications

Currently, notes on a badge or application consist of a single text field where all notes associated with the badge/application must be made.

It would be nice if we could have better notes function in Concrescent attached to applications and badges.

A note would have a:

• Unique ID
• Reference application/badge ID (e.g., A30, BA10, etc.) used to tie the note to a single application or badge
• User ID of person making the note
• User ID of the person who last edited the note
• Date/time note was made
• Date/time note was edited
• Contents of note
• Type of note (maybe?)

(Bonus points if we could see note edit history in detail, but not necessary.)
