Add migrations for previous releases #133

In Redshift v1.3.0 we introduced the idea of migration files, which contain the SQL that needs to be run once in order to migrate from the previous version.

A maintenance release to add migration files for all the previous releases should make life easier for anyone who needs to update their models.
