Registration form descriptions confusing

The user registration had lines for both a name and a last name. Name should probably be a first name. Is there a reason to include username, first, and last name all separately. One username field might suffice.

This will be fixed by next checkpoint to just have username.....no need to actual person's name for this kind of app.
