When a signal crosses from over one ground plane to another, noise may be introduced. The current PCB should be reviewed to make sure signals remain over their ground plane.

Also make sure any high-speed, sensitive signals are routed on the component side, nearest the ground plane. Route a ground trace by any sensitive signals on the solder side. Make sure to flood open space with ground.

Upgrade filter caps to lower ESR.
Make sure proper capacitor type is specified for C25.
