Create withdraw earnings modal #196

Create a modal where the user will be able to withdraw his earnings from royalties and song sales. The modal should include:

[x] A title that says Withdraw Earnings
[x] Instructions to select songs from which the user wants to retrieve earnings
[x] a checkbox to select the songs, hopefully with a select all option
[x] Instructions to accept all transactions in MetaMask when the pop up shows
[x] A withdraw button to activate MetaMask
[x] A cancel button to exit the modal

Upon successful submission, the modal should close
