add documentation on how patches_base is calculated #109

this documents how patches_base is calculated, including the special handling of the milestone macro, as requested in #108.

Is it? The README simply says "Please use the github Issues to report bugs.", and doesn't address code submissions. Maybe after merging this PR someone can fix the README.
