pixelation issues with rgb2hdmi board #5

I have installed the rpg2hdmi board below in my Rev 5 Amiga 500. The display has incorrect pixels sprinkled over the display and some of them flash. I also noticed the display can change when I press on the Denise chip. I could order some contact cleaner if it is a connectivity issue. Alternatively, are there settings I should be adjusting to clear this up?

Where did you purchase this RGBtoHDMI board from, and which exact model/version?
