Overview

This pull request:

Refactors the cassandra-driver probe to make it stylistically similar to the other "SQL probes".
Refactors and simplifies test setup.
Added SQL Trace Context injection as in to the other "SQL probes".

Pull Request merges into nh-main. Agent built from master will stay unchanged.
