I can't believe that such an awesome app still have no wiki page.

You can check your gmail setting for the two-step verification. Then, when you 
choose to turn it on, Google will verify your phone number (via SMS or Voice 
Call).

Now, you'll move on to a page with mobile application settings. You can choose 
Android, iPhone, or BlackBerry. There's no place for java-based phone or Nokia. 
So I choose BlackBerry.
The most important information is the secret code on that page.

Now press the joy stick on your phone(For me, this is an E72, so maybe you'll 
need to find your own key), then input the secret code just given. Done! You 
can verify with the time-based code generated by this app now.

you also need to input "account name" ...I presume this is without the @ sign...

 but then I can´t get it to accept a code even though the time matches...

presumablelt this is not an option:

"If you would like verification codes that are counter based instead of
timebased, use the "google-authenticator" binary to generate a secret key in
your home directory with the proper option.  In this mode, clock skew is
irrelevant and the window size option now applies to how many codes beyond the
current one that would be accepted, to reduce synchronization problems."

I also presume this is not possible: 
"The PAM module has an optional feature to detect and compensate for excessive 
skew in time stamps. If enabled, if requires the user to repeatedly try to log 
in. After a sequence of three successful tokens the PAM module learns the 
client's time skew and will subsequently compensate for it. See the README file 
for details."
