Can not view my form on my HP after having uploaded it on Ona. #1107

What steps will reproduce the problem?
1. I was able to see the form on my phone up to yesterday 
2. I am making changes in my form every day but nothing very special
3. Today, after making more changes, and being able to download it on Ona, I 
get a message error when I want to view it on my phone(unfortunately, ODK 
collect has stopped".

What is the expected output? What do you see instead?
I see the error message saying ODK hs stopped,

What version of the product are you using? On what operating system?
All the info is in the attached docs (I used Cat Log)

Please provide any additional information below.
