Repeat group prompts at begin and end statement

Hello everyone,

I'm a new user and I built an Xform using Excel to be used on android mobile 
devices. My Xform previews fine in the web browser but once loaded into ODK 
Collect on my HTC One (Android version 5.0.2) I found an issue. I tried 
searching for the issue but didn't find anything. 

The issue is with my repeat commands in the form. When filling out the form on 
my HTC one, the form prompts to add another group twice, first at the 
begin_repeat statement and then at the end_repeat statement. In other words, it 
asks if you want to add another group before you can even view the questions in 
the group, and then if you select don't add another group, all questions within 
the group are skipped. If you select add another group, you can then view and 
answer the questions in the group, followed by a second prompt to add or don't 
add another group. I'm sure it's probably a newbie error in how I built the 
form but I can't figure it out. Many thanks in advance. Attached is the Excel
