[Wizard] [Enhancement] Add a new SIP Provider Wizard (Modulus)

Hello,

I'd like to request the inclusion of a Wizard for Modulus, an Athens, Greece 
based VoIP operator which holds a licence issued by the Hellenic 
Telecommunications and Post Commission.

Here's the settings for this provider:

1) Fields provided by the user:

Username: USERNAME (text keyboard, can include any alphanumeric)
Password: PASSWORD
Display Name: DISPLAY NAME

2) Fields provided by the wizard:

Account Name: Modulus
Account id = <sip:USERNAME@voips.modulus.gr>
Registration URI = sip:voips.modulus.gr
username = USERNAME
Data (password) = PASSWORD
reg timeout = 600
Proxy URI = sip:voips.modulus.gr
Voice mail = 13000
Subscribe MWI = DISABLED

All other settings are to be set to default. The service is available 
worldwide, although only greek DIDs are offered at the moment.
