After dialling number or select from contacts, long press on Send Key or dialer
choice menu, dialer will dial out number and enable speaker phone.

Very interesting idea.

About auto handfree I was also wondering if another feature wouldn't be nice
(It's in my mind since a long time but didn't tried to implement it yet).
Let me know what do you think about that :

I can actually detect thanks to proximity sensor whether the user's face is
near or not of the phone. Assuming that the proximity sensor works well, it
could make sense to automatically switch to speaker if face is far from the
screen and to earpiece if face is near to the screen.

I didn't see any phone nor sip app that do that (or at least provide an option
for that). They probably have a good reason to, but I don't know what it could
be...

As an user when I put my face far to the screen it's usually to dial dtmf
digits or to switch to handfree mode or to hang up.
For the two first point automatic speaker mode is what I actually want (most of
the time when I dial digits I also turn speaker on to be able to ear what's
said by the auto attendant).
For the last point, I don't think that it would be too much hurting to switch
to speaker cause sometimes the remote party has a last thing to say and
sometimes it's cool to hear what he says too :)

Of course that should be an option by default to off. But maybe could be
interesting to have. What do you think?

Another solution could be to implement as HTC does with recent devices : they
use the compass sensor + proximity to automatically activate speaker if device
is returned and laid on a table to switch to speaker phone. I could do that too.

Using Promixity could be another very new and first of its kind dialer
in the world.

by using long press also be good but enable speakphone manually. Older
HTC device has this feature. And this feature is good for Phones
without proximity.

Handfree is good for motorist during driving. Touchscreen device is
already not motorist friendly by default. It not easy to dial from
touchscreen during driving. Voice command dont work so well especially
names of different languages.

You could have proximity feature available but not by default.
You can make use of this proximity feature to enable handfree when it
is away from any object, especially for those who dont want to hold
the phone while waiting to get the phone connected or motorist placing
the phone on the dashboard. And when phone is placed near the face,
disable handfree and enable back handfree when phone is away the face
again. I think feature should be the first of its kind dialer in the
world. Should patent this feature as well.
