formatting: more details in PR output #69

  1. Include author's name if merged PR was not created by the merging user
  2. Include repo owner's name in refs if PR head branch is in a fork

I started on this because of #25 but it looks like @half-duplex meant something else by that issue. Did it anyway. 😁
