Refactor operation modelling templates #78

What new or enhanced feature are you proposing?
Refactor operational modelling into a new template folder; one for each
concept (data flows, data sources, chained operations).

Make operation output unified - all templates have a parameter 'isClientSide'.

What goal would this enhancement help you achieve?
More reliable and testable codegen templates.

Unify both client-side code and server-side code into the same codegen 
templates,
linked by the 'isClientSide' template parameter.

Resolved in r914.

All PHP/JS templates are now unified with a 'Boolean php' parameter.
