I don't know yet if this is possible. with adjustable controls that are a percentage, when using Voiceover swipe up or down usually adjusts by 10%. I agree that is quite a large change.
It is possible to make fine adjustments with Voiceover though: double tap and hold, then slide left or right. Lift your finger when the desired percentage is reached. This works on most controls that Voiceover reports as adjustable.
From a Soundscape Community user... There is an issue with VoiceOver speech speed when Apple introduced IOS 17 and which has become worse with release of 17.4! The problem is when the VoiceOver overlay is applied the 50% speed becomes probably 46% on iPhone and iPad and ironically speeds up on Apple TV. This causes issues with third party apps like SoundScape, as now the voice speed is compromised further within SoundScape. Currently the Blind Voice Speech Speed can only be adjusted at 10% increments, this means the SoundScape speech is slow as molasses at 50% and way fast at 60%. The best solution here would be for Apple to fix the problem and stop corrupting VoiceOver every time a new IOS release update occurs, but the realistic fix will probably need to come from application developers. A suggested fix is to implement 1% increments in lieu of 10% increments when setting Blind Voice Speech Speed as this would totally resolve any future issues relating to Speech Speed.
