The Dense Ores mod may be using a custom vertex format, which is not compatible with shaders. Only BLOCK and ITEM formats can be used with shaders.
Alright. Thanks for pointing me in the right direction. I'll bug the author of the Dense Ores mod.

Running Minecraft 1.10.2 with Forge 12.18.6.2297 and OptiFine_1.10.2_HD_U_D8
Textures from this mod are garbled only when using shaders, regular OptiFine renders correctly. In the screenshot, I am holding dense iron, however all ore blocks are scrambled in a similar fashion. I've also attached my run log.
There is an issue open on the Dense Ores dev's page as well. Not sure if the bug is on their side or yours.
