Please provide any additional information below.
Works fine in SSP, worked perfectly fine in SMP on my tekkit server, then at
random clients are crashing, all giving this error, no idea what the hell is
going on, went through and deleted all of the pipes on the map with mcedit
search replace(replaced with air) started server back up, dropped all three
teleport pipes, crashed my client on each one of them with this same error.

Please provide any screenshots you feel would help demonstrate this
issue/feature.

I have a similar issue, however, it's a problem with tekkit SMP (on the server
itself) - examine the server log and I bet you'll see this:

[FINER] Adding mods from
/home/minecraft/tekkit1.1.4/mods/buildcraft-server-DA-additionalpipes-31-41
(1).zip
[FINER] Zip found.
[FINE] Warning: mod_zAdditionalPipes$chunkXZ.class is in the wrong folder
(won't be loaded).
[FINE] Warning: mod_zAdditionalPipes.class is in the wrong folder (won't be
loaded).

Gonna keep testing.

also having this issue.

logged in a few times, seems to be affecting all Teleport pipes, wierd coause I
was in game, changed on to a different frequency, went to its counterpart to
turn it on and crash. now none of them work. I recently updated forge to
1.3.3.12 but that was quite a while ago.

I have the same problem, except in the server log, it just says the cause of
the player getting removed was "disconnect.endofStream". Sometimes the pipes
crash the client, sometimes they don't. Nothing appears in my error log, which
is why it is so frustrating. Please fix this issue!

When I first installed my teleportation pipes, they worked just fine, but when
I wanted to move my quarry a little while later, right-clicking the pipes
caused my client to crash. Turned out, the problem was that I was an op by
then. After de-opping myself and restarting the server, everything was OK
again. Hope that helps!

I can confirm the affirmation of the comment number 5. Deopping yourself and
rebooting the server worked.
