What steps will reproduce the problem?
1.placing a new teleporter pipe waterproof/powered

What is the expected output? What do you see instead?
items in inventory/hands vanish after placing any of the teleporter pipes SMP
they come back after disconnect and reconnect

What version of the product are you using? On what operating system?
1.6

I looked into this and can't seem to replicate it. However I did notice that 
when I am lagging alot Minecraft can do this to me when doing everyday things 
(Such as placing items). Do you have a server I can connect to to test it?

seems to work fine in 1.7 havent had any issues with 1.7
