So, this will be 2 issues in 1 :)

My up-to-date setup:

Firefox 36.0 with default skin
Toolbar Autohide 6.6.1
(not neccessary: FireGestures 1.8.7)

...plus one of any tab-manager:

Tab Mix Plus 0.4.1.6
Tab Utilities 1.5.28.1
Tab Kit 2nd ed. 0.12.0

[hint] You can switch on/off titlebar by Customize/Title bar button

[pre] Set Your preferred tab-manager's max. rows to 10 and open as many tabs as neccessary to fill 10 tab bar rows to clarly see the size of the bad effects. If You do not using multi-rows, autohiding works well.

Steps to produce good -> wrong behaviours:

The lucky case:

[A1] Turn Title bar on (see [hint]) [A2] Set Maximized & Unmaximized Autohide off [A3] Maximize, close & start Firefox [A4] While tabs are preloading, Tab bar appears with addressbar and such [A5] Turn Maximized Autohide on [A6] Autohiding works well

The unlucky case:

[B1] See [A1] [B2] Set Maximized Autohide on, Unmaximized Autohide off [B3] See [A3] [B4] While tabs are preloading, Tab bar appears without addressbar and such [B5] This bar stucks on-screen and You can not hide [B6] Turn Title bar off then on and voilá: [B7] See [A6]

The ugly case:

[C1-C5] See [B1-B5] [C6] Turn Title bar off [C7] Bars not appears by mouse hover at top [C8] Move mouse to bottom of Firefox [C9] Press & release Alt key twice [C10] A blank 10 rows tall area appears at very top

Dissolving the blank area:

[D1] See [C8] [D2] Press & release Alt key to display Menu bar [D3] Move mouse over any bar (Menu bar preferred) [D4] Press & release Alt key [D5] See [A6]
