feat: a clean up (move to standard, pre-commit hooks, more travis testing and coverage)

• [x] set up travis to run more versions of Node.js
• [x] update of package.json to new urls
• [x] move to StandardJS
• [x] pre-commit hook
• [x] run coverage on tests

As expected, tests fail on Node.js 4 or above (as pointed out before on: https://github.com/spdy-http2/http-deceiver/issues/2), this PR has nothing to do with it, will merge and investigate separately.
