Instead of changing this in the DB, we should create a page where you can create questions and assign those to a question set which can be used for Retrospectives.
I suggest the following could work:
Can create/edit/delete a question
Can created/edit/delete a question set, which has at least 1 question and no limit on the max number of questions. A question set has two sections "Project Health" and "Project Smells Check" and a question can be assigned to one of these sections (these are displayed as different sections in the form. Note - a piece of work in the future could be to enable section creation but I don't think this is needed right now)
Can select a question set to use when creating a retrospective.
If there is only a single question set, then there is no select option when creating a new retrospective.
Once a question set is applied to a retrospective it cannot be changed (the retrospective will need to be deleted and a new one created)
Once a question set is applied, editing the questions or the question set does not affect that retrospective.
This should be: "Change is well managed"
The description should be: "When the project changes, the change is handled with appropriate workflows and is not a knee jerk reaction".
Note - this change can be done in the database and any retrospectives with the existing "churn rate is low" question should stay as they are.
