Include Skip-Reason in output-file #17

This PR could solve #16 .

Please have a look if there is room for improvements as I don't have that much experience with the vs-test-platform:

I'm not sure if it is okay to introduce own TestResultMessage-category for this.
It also depends quite a bit on the output of the test-runner to always have full name in message for skipped tests and ending with [SKIP] as well as to emit the reason immediately in the following message.

The appveyor-test fails on the newly added acceptance-test but i cannot see why. It all works as expected locally but on appveyor, the generated file obviously doesn't have the <reason>-Element...
