AttributeError: Dataset does not have attribute 'DVHReferencedROIs'. #61

Although the DVHReferencedROI sequence is a required element, some RT DVH 
modules don't include it.

The fix is to skip this DVH if the DVHReferencedROI sequence is not present, 
and let dicompyler calculate the DVH, since we cannot determine what 
structure/ROI the DVH belongs to.
