Full list of changes:

Move spiffe-helper.go to pkg/ directory and make it part of spiffe package rather than main
Move main.go and config.go to cmd/ directory
Add ExternalProcess flag to signify process will be managed externally
Add ProcessReloadFunc to specify external function that will be called to reload process when new cert is received.
