Adds homepage and reformats summary for better `gem list -d` output :) #1

This plugin is an amazing idea! Really helps to minimize and standardize a vagrant box by covering an area that is often neglected

I just thought the gem list -rd formatting made it look a little neglected, so I thought I'd flesh it out

In case it's helpful, I tried to clean up and clarify the README. I found there were a few details that were mentioned multiple times, and some things that were mentioned in places where it wasn't particularly relevant. (I was working under the assumption that the README only need to have enough to use the plugin, and if someone is interested, they'll look into the code to see how it works :)

It's no problem if you'd rather not accept that change.
