'Undefined array key 4' warning when submitting a form with PHP 8.1+ #318

This PHP warning was previously reported in #312, but since that issue also reports other, unrelated warnings on the Mailer class (which I do not use), I'm opening a separate issue.

Pull request follows.

This is probably be the same root cause as the first 2 warnings reported in https://github.com/splitbrain/dokuwiki-plugin-bureaucracy/issues/288#issuecomment-786614423
