Compatibility with cassandra 2.1.13 #141

I've based these changes on #121 and resolved the conflicts in the process. I've also bumped up the cassandra version to 2.1.13.

I'd like to test this out. Can you point me to the source branch? You may need to re-do the PR, because source branch shows up as "unknown repo".

Incremental repairs & better 2.1 support are quite useful. Any chance for this not not end up as https://github.com/spotify/cassandra-reaper/pull/121?
