Fix "Save & Continue" + Payment Profiles. #102

Previously, the save & continue button would not be removed when combined with certain payment profile options.

Thanks @gmacdougall! I've added this to 1-3-stable, 2-0-stable, 2-1-stable, 2-2-stable and master.

looks like we listen clicks on every payment method and then check if paypal is checked? i think this might not play well with other payment methods that wants to hide the button as well with the same approach
