complete upgrade to spree 1.0 #10

I haven't tested this yet, but I copied all the artifacts generated by 'spree install', and updated the README to reflect the new way of installing the migrations.

I'll close this one for now as the master branch diverted a lot with https://github.com/funkensturm/spree_digital/issues/8
