Issue Description: User Registration 'Risk Assessment' issues

Steps:

1. URL: http://test.sprim-health.com/
2. Click on 'I'm a User'.

AR:

1. Mandatory fields are not showing in * mark.
2. Help text is showing only YY for year.
3. Year drop down is showing only till 1995.
4. Mouse hover button color is showing in White Color.
5. Pounds is not allowing to enter more than 3 characters.

ER:

1. Risk Assessment: -> Mandatory fields should display in * mark. -> Year help text should display as YYYY. -> Year drop down should allow to select above 1995. -> Mouse hover button color should not be in White Color. -> Pounds should allow to enter minimum 6 characters. (eg: 154.25)
