Finish php --> NodeJS conversion #48

Three parts: 1) Validate there aren't any issues with leaders_node.html and standings_node.html 2) Convert hof.php (just some more sql queries) 3) Improve performance (possibly related to stored queries)

1 is done. 3 doesn't seem to be an issue, but stored queries are a later task for quality assurance. Working on 2 now.
