auto-linking should check for empty `name` in addition to `id` #6

Per title, otherwise anchors like <a name="foo">...</a> would be subject to auto-linking.

(To be sure that other similar cases don't occur later, perhaps it would be best to whitelist -- e.g. only allow class -- instead of blacklisting?)
