improve error messages #91

This patch improve IVM error message format and add errcode.

The format is simply unified, so please let me know if you have any comments.

Although "this query is not allowed on incrementally maintainable materialized view" can be more improved, I don't have a good idea for now. So, I'll merge this now and fix if we get something better.
