multiple playOrder with same value (which is allowed) causing problems #152

NCX (and by inheritance OCF/ePub) allows the same playOrder value to be 
used more than once so long as the target content is identical.

It seems Bookworm is having difficulty properly building the ToC when this 
occurs.
