Support NCX page list and associated page references #174

e.g.

When the NCX file has pageTarget data, there should be a way to go to a
particular page (e.g., to look up a reference to a page number in a dead
tree edition of the book) -- and you should be able to construct an URL to
the appropriate page knowing only the page number (and the base URL for the
book), not the internal page and link target names.
