Remove `@type="supplied" from titles (except for "Unspecified contents") #54

All English titles should have a @type="supplied" attribute.

Cf. #75 which might link to different title data formats for find-and-replace regexs.

@davidamichelson we should check in about our preferred title format (required attributes, optional attributes, allowed child elements, etc.). This data is all over the place, so I want to get on the same page before doing any normalization work.
