Create a report of author names and uniform titles for proofing #180

Splits off from #177 . Will create a spreadsheet with two primary columns: "Author" and "Uniform Title". Then the editors can find any typos, duplicates, incomplete name forms, etc. The result will be a normalized list of works by author and title that can then be used to fix current data.

We can also make this into a title and author index, which could be helpful for authority control.

I'll open a separate issue to track normalizations, etc. that we want to do using this list.
