UnboundLocalError: local variable 'form' referenced before assignment

Just documenting this. Should be easy to fix.

Added the empty form with an else clause. https://github.com/Captainkrtek/OpenRelay/commit/b1e8fff7054f2a88efe62db1632baca540c19ee8

Fixes the thing temporarily and this is minor anyway.
