Steps needed to implement this:

1) change clb view_usages to handle synonyms by a) ignoring synonyms in 
parent/child relationships, and b) appending as floating (so, 
classification-less) concept.

2) generate the relationship_assertion table, mapping any unaccepted synonyms 
(from_concept_id) to the accepted concept (to_concept_id), setting the 
relationship_type_id to the appropriate int to represent synonym
