This is a first draft for the changes to the direct messages key derivation spec. I think I am happy with the substance now, but the rationale reads a bit... rambling. I'm tired. I'll do my best to look into this in the next few days, but if you want to read through it and provide specific feedback @mixmix, that would probably also help.

Oh yes and this PR sits on top of #7, so that one should probably be merged first
